 For your convenience, you may use the form below to request an online pet prescription refill Fayetteville NC of your pet's prescription. This form is to be used only for prescriptions that you will pick up at the clinic.
Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor.
We will email or call you if there are any questions or problems with your request. Please note that prescription requests via email are checked Monday through Friday.
